La Red Health Center
participates in most of the local employer-sponsored
insurance plans, as well as Medicare and Medicaid. For
patients who do not have insurance, La Red Health Center
assesses fees based on a sliding fee scale which is
predicated on annually revised federal poverty
guidelines.
Prenatal
and Perinatal Services: Pregnancy tests are offered
to sexually active women and those who test positive are
referred into the prenatal program. La Red Health Center
contracts with a private obstetrical group for onsite
prenatal and postpartum services, as well as labor and
delivery services at a local hospital. Through
collaborations with other local organizations, the prenatal
program provides onsite nutrition counseling, parenting
classes, and gestational diabetes education and management
as additional education components. La Red Health Center’s
Promotora Program (case management) facilitates the
enrollment of low income pregnant into assistance programs
such as WIC (food assistance) and public health coverage.
Children and Adolescents: La Red Health Center
administers an immunization program, participates in the
State’s Vaccines for Children’s program, completes
developmental assessments, and provides nutritional advice
and education to families, including referral to public
programs for food assistance. Services for adolescents
currently include developmental assessment, physical
examinations for school and sports, and health education
regarding sexually transmitted diseases.
Adults: Primary, acute and chronic disease
treatment is conducted by a provider team who completes a
detailed review of age and gender appropriate risk factors
for diabetes, coronary artery disease, cancer, sexually
transmitted diseases, behavioral health issues, and other
medical conditions. Diagnostic procedures and subspecialty
care are referred to physician partners as appropriate.
Geriatrics: Screenings are available for
osteoarthritic conditions and osteoporosis. Immunizations
are offered for preventable illnesses such as influenza (age
50 and older), pneumonia (age 65 and older), and tetanus
(all adults every 10 years). Other preventive screening
services for cancer, diabetes, mental health are routinely
administered. Free glaucoma screening is available on a
periodic basis through a local partnership.
Mental Health and Substance Abuse Counseling,
conducted by a bilingual Board certified psychiatrist, is
made possible through a contract with Delaware’s Division of
Substance Abuse and Mental Health to meet an unmet need for
services generally in Sussex County, but specifically as
they relate to culturally appropriate care. The psychiatrist
addresses issues ranging from postpartum depression to
domestic violence, to depression, to substance and alcohol
abuse.
Oral Health Services are available to existing
patients at a reduced fee through a collaborative agreement
with a Bureau of Primary Health Care funded program.
Emergency Medical Services are available to
patients 24 hours per day/7 days per week After hours calls
are automatically forwarded to a cell phone carried by the
medical director.
PREVENTIVE
HEALTH SERVICES
Prenatal
and Perinatal Services provides vital services and
education with the intent of increasing the rate of care
offered to pregnant women in their first trimester of
pregnancy and lowering the rate of gestational diabetes, low
birth weights, and infant mortality.
Cancer Screening is available through La Red
Health Center’s participation in the Delaware Division of
Public Health’s Screening for Life Program. The program
provides free breast, cervical and colorectal cancer
screening exams for uninsured eligible women (up to 200%
FPL) (and men for colorectal) over the age of 18 years.
Well-Child Services include routine school and
sports physicals
Immunizations against Vaccine-Preventable Diseases
provides age-appropriate immunizations for all life cycles
including diphtheria, pertussis, tetanus, polio, measles,
mumps, rubella, and influenza.
Blood Screenings are offered at no cost for
cholesterol and osteoporosis through two partnerships with
local hospitals. Lead poisoning screening is also available.
Small Employer Assistance is available to
those who wish to offer pre-employment screening or routine
or customized annual physical examinations to their
employees.
ENABLING
SERVICES
Transportation Services
are currently performed by two bilingual outreach
workers/case managers. By providing such services, it is
assured that critical continuum of care referral services
are completed and that no-show rates are minimized for other
service providers.
Translation/Interpretation Services are
conducted by four certified medical interpreters who
provide services for patients who have made appointments at
other medical offices where resources are not available. The
staff of La Red Health Center is fully bilingual and patient
care is provided in a culturally competent manner.
Prescription Assistance is available on a
limited basis to assist low-income, uninsured patients who
qualify for discounted fee-for-service medication programs.
PATIENT
EDUCATION & OUTREACH SERVICES
Diabetes Prevention and
Management services are directed by a Certified
Diabetes Educator who is funded through the Delaware Health
Care Commission to provide onsite diabetes screening and
nutrition counseling.
HIV Prevention Outreach, Testing
and Counseling
services are conducted a coordinator who has been trained
and certified by Delaware’s Division of Public Health to
provide health education, risk reduction training, disease
screening, counseling and case management referrals as
required.
Childhood Immunization Outreach is conducted
to educate parents regarding the importance of adhering to
prescribed schedules of immunization and to assist them in
obtaining records from the State’s databank to facilitate
school enrollments.
