Chronic Disease Management
At La Red Health Center, our Care Coordination Team provides personalized support to help patients manage chronic conditions and stay on track with long-term health goals. This dedicated team of compassionate nurses works closely with patients—especially those with complex or ongoing needs—to ensure they receive the care, education, and follow-up support they deserve.
Our chronic disease management services include:
- Medicare Annual Wellness Visits
- Assess overall health, identify potential risks, and develop personalized care plan for senior patients. Focus on prevention and long-term wellness. Patients also have access to our 340B discount pharmacy program.
- Free and low-cost cancer screenings through Delaware’s Screening for Life Program.
- Resources and support for chronic disease management, from diabetes and high blood pressure to heart disease and more.
- A team that acts as advocates and partners, helping patients overcome barriers to care and empowering them to take charge of their health.
