At La Red Health Center, we believe that everyone deserves compassionate care, including those experiencing homelessness. Our dedicated Case Managers go beyond just providing services — they build trust, offer support, and connect individuals with the care they need to improve their well-being.

Here's how we make a difference:
- We meet people where they are — whether in schools, businesses, churches, or community gathering places, we offer them help and support.
- We take the time to listen and understand each person’s unique needs, and create a plan that puts them on a path to stability and health.
- We assist with gathering medical and job history and scheduling essential health care appointments.
- We provide ongoing support through visits, phone calls, and home check-ins, ensuring no one feels forgotten.
- We help with transportation to the health center, so access to care is never a barrier.
- We advocate for our clients, making sure vital resources reach those who need them most.
- We stay informed and collaborate with community partners to find the best ways to support those facing homelessness.
- Most importantly, we welcome everyone with dignity, respect, and open arms — because at La Red, you are never alone.
Our mental health team also collaborates with doctors and nurses to create a full care plan for each patient experiencing homelessness.
If you or someone you know needs support, we're here to help. You can access our behavioral and mental health services at any of our care centers in Georgetown, Milford, or Seaford.
If you are a new patient or don't have access to your patient portal, you may also schedule an appointment by calling or visiting the care centers closest to you.
