Case Management for the Homeless Population

At La Red Health Center, our Case Manager works to meet the immediate needs identified by the homeless individual while establishing a trusting relationship and gradually engaging the homeless individual into the comprehensive health care services that LRHC provides.

Specifically, LRHC's Case Manager for the homeless conducts community outreach in schools, businesses and churches and outreach into areas where high risk for homeless populations reside and congregate, and performs the following functions:

Conducts intake interviews to assess the needs of the client.

Plans a course of action to address identified needs.

Obtains medical and employment history and sets up medical appointments as needed.

Sets and observes appropriate boundaries with clients.

Ensures resources are used by eligible households who are homeless or at the risk of homeless.

Maintains & documenst regular contact with clients: health center visits, telephone contact, and home visits.

Provides transportation, as required, to the health center, or other required service agencies.

Attends appropriate local, regional, and national meetings on homeless issues to seek out new information and best practices.

Participates in team exercises to ensure LRHC has a welcoming environment for clients utilizing LRHC programs and services.

Consistent with LRHC's patient centered medical home approach, LRHC's mental health staff organize and participate in routine clinical case conferences involving doctors, nurses, and other providers to ensure that a comprehensive health care plan is put in place for each homeless patient.
