Dentist

The Dentist will assess patients' oral health; formulate comprehensive dental treatment for patients across the age spectrum; implement dental treatment plans including emergency treatment, disease control, restoration, and recall; make referrals to specialists as needed; encourage patient participation in prevention activities. 1. Diagnose and treat diseases, injured and malformations of teeth, gums and related oral structures, and provide preventive and corrective services. 2. Examines teeth, gums, and related tissues, using dental instruments, x-rays, and other diagnostic equipment to evaluate dental health, diagnose disease or abnormalities and plan appropriate treatments. 3. Administer anesthetic to limit the amount of pain experienced by patients during dental procedures. 4. Delivers direct patient care to patients. 5. Formulates plan of care for each patient and communicated that plan. 6. Educates patients on oral health care and preventive care. 7. Using Dentrix, documents dental history or complaints, records and reports pertinent observations, treatments and care plans. 8. Conducts patient triage for walk-in patients. 9. Maintains and updates dental supplies and instruments as needed. 10. Assembles and evaluates dental health educational materials. 11. Trains and monitors performance of clinic staff. Job Requirements: " Possession of a current license to practice dentistry in the State of Delaware. " Minimum eight to ten years general dental experience; public or non-profit environment preferred. Family Practice Nurse Practioner

Licensed Practical Nurse

Under the supervision of the Lead Nurse, this position is responsible for clinical activities involving routine office nursing care and patient flow, patient recordkeeping and follow-up, patient education, and other related activities as required.

Essential Responsibilities:

1. Routine nursing duties including preparing patients for examination: check vital signs, weight, and height. Perform standard clinical and laboratory procedures and routine venipuncture, and other screenings in accordance with established clinical protocols.
2. Administer patient injections and vaccines as directed by the clinical providers.
3. Perform routine vision screening as appropriate.
4. Practice safety, environmental, and/or infection control methods.
5. Maintain exam rooms in a clean and orderly manner.
6. Update patient medication list as appropriate.
7. Complete past medical, social, surgical, and family history on every new patient.
8. Update immunization records for every child visit; research immunization records in state database as required.
9. Complete child physical forms and other physical forms as required by outside agencies.
10. Complete pregnancy visit paperwork in advance (complete pregnancy test, certification of positive pregnancy test, prescription)
11. Keep rooms stocked with requisite medical supplies, table paper, etc.
12. Prepare equipment and ensure that medical equipment is in working
13. Perform triage and telephone response to patients as assigned.
14. Call patients with lab results, and other reports as requested by provider.
15. Call in medications to pharmacies.
16. Ensure that all diagnostic and laboratory reports are collated with the appropriate patient chart and available to the respective provider at the time of each patient's visit; file associated documents in medical record as required.
17. Schedule appointments for patients as needed.
18. Educate patients and/or families as to the nature of disease and provide instruction on proper care and treatment.
19. Assist indigent patients by completing pharmaceutical applications where applicable
20. Other duties may be assigned as necessary.

Ability to effectively present information and respond to questions from groups of managers, patients, physicians, and the general public. Must possess excellent interpersonal, customer service, and telephone skills. Mathematical Skills : Ability to calculate figures and amounts such as proportions, percentages, area, circumference, and volume as they relate to patient information and medication dosages. Ability to apply concepts of basic algebra and geometry. Reasoning Ability : Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations : Must possess current license as a Licensed Practical Nurse, valid in the State of Delaware. All mandatory competencies demonstrated annually. Promotoro/Community Health Outreach Worker

A member of a team that provides mobile and site-based outreach services to patients of La Red Health Center. Such services include but are not limited to: engagement and referrals for basic provisions, referrals to programs including public assistance and the Community Healthcare Access Program (CHAP), crisis intervention, problem solving within the system of care, treatment referrals, participation in the development of individual services plans, transportation assistance, and the coordination and brokerage of services with community based and internal resources. This position is responsible for overseeing the documentation of all services and contacts provided to patients. This position may also participate in community forums that address relevant topics/issues regarding homelessness, mental illness, special needs housing, and other medical/social issues affecting our patient population. Essential Responsibilities: The following duties are not intended to serve as a comprehensive list of all duties performed by all associates in this classification. Listed are duties intended to provide a representative summary of the major duties and responsibilities. Incumbent(s) may not be required to perform all duties listed and may be required to perform additional, position-specific duties. 1. Informal patient and family support. 2. Advocacy for individual and community needs. 3. Assuring people receive the service they need by providing and documenting the enabling services of translation, interpretation and transportation. 4. Identifying community-based resources and other assistance programs and communicating those resources to individuals in the community. 5. Record keeping and documentation on clients served, services rendered, and follow up needs. 6. Cultural mediation between communities and health and human services. Education and/or Experience: High School diploma or GED; associates degree in Human Services preferred Two (2) years previous experience in community based outreach services preferred. Strong knowledge of Sussex county communities and highways Delaware Drivers license with clean MVA driving record. Language Skills: Bilingual skills required English/Spanish or English/French Creole Performance Requirements: Knowledge: • Knowledge of local community. • Understanding of standard use and care of office equipment and supplies. • Knowledge of EMR system, medical record documentation requirements and state/federal laws related to the release of health care information. Skills: • Service coordination skills. • Time management skills, motivated and ability to self-direct. • Capacity building skills with outreach resulting in increased access for people to the Health Center. • Translate for patients referred to specialty physicians outside the La Red Center. Abilities: • Ability to impart information and education to a wide variety of audiences; community members, health professionals, and service providers. • Ability to connect to the community • Ability to communicate effectively and develop rapport with patients and families to facilitate implementation of a care plan. • Ability to develop materials, displays, posters, power point presentations etc. announcing outreach efforts and events. • Ability to remain calm when emergencies occur. • Ability to safely operate a motor vehicle with patient passengers. Equipment Operated: Company vehicles including 5 person vans and vehicles. Operate a wide range of standard office equipment. Computer use and proficiency required. Work Environment: Work environment varies from passenger /patient transport vehicle to businesses, schools and churches to Outreach office at Health center. Includes waiting rooms, exam rooms, class rooms, church halls and outdoor health and outreach efforts. Controlled lighting, air quality, and temperature settings when inside. Environmental hazards may include weather related exposures and unpredictable exposure to communicable diseases and biohazards. Mental/Physical Requirements: Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 pounds). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings at various sites when required. Stress can be triggered by an individual's response to workload, traffic or specific cases.
